Thank you for your willingness to make a recommendation for a member of the summer staff of Catholic Youth Summer Camp.  Please note the following directions.

The following instructions pertain to the successful completion of this form (http://www.ultracamp.com/forms.aspx?idCamp=297&campCode=cys&idF=920)  Feel free to print these instructions, or bookmark this page.

1. Create a personal account for yourself (or log in if you have already created one) This is your “Reference Account” and is not associated with the person you are recommending.
2. Select “Document Center” link on the left side of the account page.
3. Select “Counselor/Staff Recommendation”
4. Select your name to verify the Recommender listed will be associated with your account.
5. Input the student’s name to link the appropriate account. This step guarantees that the recommendation linked with your personal account will be visible from the account of the applicant.
6. Complete the form.
7. Verify the form with electronic signature when asked.
8. Log out (or complete any additional recommendation forms by selecting the appropriate option)
