Be a part of our incredible staff of Counselors and Program Staff Members to make this summer of CYSC our finest yet!  Application, open for those who will be entering 12th grade and older begins with filling out the 3 sections of our online application available the CYSC Document Center.

The process of application for service this summer begins by following the steps listed below.  If you have questions regarding application at any time, please feel free to email registration@cysc.com.

1. Create an account on our CYSC application website utilizing “ultracamp.”  If you have an account from a previous year, please feel free to use your previous account.  If you can’t remember your login info, click the link on the account login page and it will be emailed to you.

2. Complete all 3 sections of the online application and both (2) sections of the medical form.  These forms are available in the CYSC Document Center linked above.  Once begun, they are available under your personal account (click on your name) in your account page.

3. Short Answer Questions (first time applicants only): Complete the Short Answer Questions form that you will find in the document center (in the bar on the left hand side of your CYSC account page).

4. References (first time applicants only): First time applicants must request and secure three (3) personal references, including one from each of the following:

1) Parish Priest OR Parish Youth Minister
2) School Teacher, Coach, OR other School Activity/Club Advisor/Leader (Not applicable to adults age 21 or over who are not college students.)
3) Adult (age 21+) who is not a relative (Adults age 21 or over who are not college students are requested to obtain 2 references in this category.)

Please direct your potential references to our online reference form. There they will be able to fill out the form online.  (Your potential reference will have to create their own personal account that will be used to submit your registration.  They will simply indicate your name in the form that they submit through their own account.)

4. BCI deadline for completion – June 9 (or before your first day of service at camp):

If you are, or will be, age 18 by June 9, 2013, you will be obliged to obtain an Ohio Bureau of Criminal Investigation and Identification (BCI&I) background check. (Per CYSC, Inc. Policies and Guidelines)

To obtain/schedule a BCI: Contact “Fast Fingerprints” by calling 614-457-8900. You must inform them that you are with the Columbus Catholic Diocese /CYSC. All details at www.fastfingerprints.com. Download and complete “General Registration Form” and take with you to your appointment. Three convenient locations, open Monday-Saturday. Cost is $37.

5. Protecting God’s Children Workshop deadline for completion – June 9 (or before your first day of service at camp):

If you are, or will be, age 18 by June 9, 2013, you will be required to participate in a Protecting God’s Children training session prior to beginning your service with CYSC (Per CYSC, Inc. Policies and Guidelines)
